Service Fees and Insurance

I currently accept Regence and Premera. With all other providers, I am out of network at this time.  I am happy to print superbills for you to submit to your insurance provider of choice.  I also accept HSA cards.  Co-pays and service fees are due at the time of service.  Atha Wellness reserves the right to keep a current Credit Card on file to use towards any balance due.  Atha Wellness has a 24 hour cancellation policy.  No shows and cancellations after the 24 hour cancellation period will be charged the full rate.  Please be aware that my billing is done in the morning the day of the appointment. 

**Please check with your insurance provider before appointments to get information on the level of coverage for acupuncture treatments and how to seek reimbursement.  Additionally, not all parts of treatment may be covered by your insurance company, for example:  the hypno part of hypnopuncture, cupping, moxibustion, injections, manual therapy, LED light therapy, tuning fork therapy, etc.  All treatments are a collaboration between the practitioner and the patient, and by signing the consent form, you acknowledge your financial responsibility towards  the balance of your treatment.
